Cave-ins. This is the big one. Unprotected trench walls can collapse without warning, burying workers under tons of soil. Cave-ins cause the majority of excavation deaths.
Falling loads. Material handled by equipment, or spoil piles stacked too close to the edge, can tumble back into the trench and strike workers below.
Hazardous atmospheres. Excavations can trap dangerous gases, low oxygen, or toxic fumes, especially in deep trenches or near sewer and utility lines.
Water accumulation. Standing water weakens trench walls, hides hazards, and raises the risk of drowning and collapse.

Stable rock. The most secure, capable of standing vertically.
Type A. Cohesive soils like clay, with the highest stability among soil types.
Type B. Moderately stable soils, including some silts and disturbed materials.
Type C. The least stable, including sand, gravel, and soil with water seeping in.
Correct classification decides which protective system you need. Get it wrong, and the protection may not match the risk.
Protective Systems
OSHA requires a protective system for trenches five feet deep or greater, unless the excavation is made entirely in stable rock. Trenches 20 feet deep or more require a system designed by a registered professional engineer. Your main options include:
Sloping. Cutting trench walls back at an angle based on soil type, so they won't collapse inward.
Benching. Creating a series of horizontal steps in the trench walls, used with certain soil types.
Shoring. Installing supports, often hydraulic, that press against trench walls to prevent movement.
Shielding (trench boxes). Using protective structures that shield workers from a collapse, even if the walls give way.
The right system depends on depth, soil, water, and nearby loads. The competent person ties it all together.
Access, Egress, and Daily Conditions
Subpart P also requires safe entry and exit. Trenches four feet deep or more need a ladder, ramp, or stairway within 25 feet of every worker. Spoil piles and equipment must sit at least two feet from the trench edge. And atmospheres must be tested where hazardous conditions could exist.

Put these into action on every job:
Call before you dig. Locate and mark underground utilities before breaking ground. This single step prevents some of the worst excavation incidents.
Keep the load back. Stage spoil piles, materials, and equipment well away from the trench edge to reduce surcharge loads.
Never enter an unprotected trench. No task is worth the risk. If the protective system isn't in place, no one goes in.
Test the air. In trenches where hazardous atmospheres are possible, test before entry and provide ventilation when needed.
Control the water. Remove standing water with proper equipment, and reassess wall stability after rain or any water intrusion.
Maintain clear access. Keep ladders or ramps within reach of every worker at all times.
These practices become second nature when your team understands why each one matters.
Inspection Protocols That Catch Problems Early
Conditions in and around a trench change constantly. Inspections are how you stay ahead of the danger.
The competent person must inspect:
Before each shift starts, examining the excavation, protective systems, and surrounding area
As needed throughout the day, especially as work progresses and conditions shift
After any event that increases risk, such as rainstorms, vibration from heavy equipment, or signs of soil movement
During each inspection, the competent person looks for warning signs: cracks in the trench walls, water seeping in, soil sloughing off, tension cracks near the edge, or shifts in the protective system. If anything raises concern, work stops until the hazard is corrected. Documenting these inspections also protects your team and proves your commitment to safety.
